RELEASE AND WAIVER

 

In consideration of the admission as a member and permission to participate in the practice of boxing, kickboxing and Muay Thai granted to me (or) granted to my son / daughter / ward BARBU KICKBOXING INC, I hereby indemnify BARBU KICKBOXING INC, its coaches, officials, members, agents, officers and employees, against any recourse of my heirs, executors, administrators of the estate or assigns, in the event of bodily injuries, known or unknown, and property damage related to the practice of the sport of boxing, kickboxing and Muay Thai. I, the undersigned, further declare that I am fully aware that these sports involve risks, but that I assume them myself and waive all possible recourses indicated above.

 

I consent to BARBU KICKBOXING INC recording and distributing photo or video images in which I appear to help promote its activities. I affirm that the information I have provided is true and complete. I am aware of my obligation to ensure that the information is updated if circumstances change.
